Your child may be participating in the following activities: Transport: Assist in transporting dogs from foster to adoption events, veterinary appointments, etc. Exercise and socialization: opportunities include taking a dog living at a foster's on day field trips to give them exposure to new surroundings, group pack hikes, and other approved activities. All dogs will remain on leash at all times. Experienced handlers only. Video and Photography: Record and edit video and photos of Cause dogs for marketing purposes. Adoption Events: Assist at mobile adoption events, may include set-up and breakdown, dog handling, interacting, transporting dogs and assisting public. Have you read and do you understand the potential activities your child may be participating in?

As a volunteer/participant of Cause for San Bernardino Paws, I recognize and acknowledge that there are certain risks of physical injury to my minor child and I agree to assume the full risk of any injuries, including death, damages or loss which my minor child may sustain as a result of volunteering/participating in any and all activities associated with Cause for San Bernardino Paws. I do hereby release and discharge Cause for San Bernardino Paws, their officers, agents, servants, and employees from any and all claims from injuries, including death, damages or loss which may occur to my minor child on account of his/her volunteering/participating with Cause for San Bernardino Paws. I further agree to indemnify and hold harmless and defend Cause for San Bernardino Paws, their officers, agents, servants and employees from any and all claims resulting from injuries including death, damages and loss sustained by my minor child arising out of, connected with, or in any way associated with volunteering/participating with Cause for San Bernardino Paws. By selecting yes, I agree to the terms of this Waiver of Liability.

During my minor child's volunteer time with Cause for San Bernardino Paws, I understand his/her photo may be taken at various events. By selecting yes below, I also hereby grant Cause for San Bernardino Paws permission to use my minor child's likeness in photograph(s) and/or video(s) in any and all of its publications or online, whether now known or hereafter existing. I will make no monetary or other claim against Cause for San Bernardino Paws for the use of the photograph(s) and/or video(s).
